Our Ethos

Moss Farm operates as a specialised, Childrens home, designed to support children aged 8 to 16. Our focus is on providing assistance for planned admissions while minimising disruptions to the young person’s life.

Upon contact, our goal is to collaborate with the Local Authority for a swift assessment and planning process. We work closely with them to devise a care and placement plan tailored to the individual needs of the young person.

Throughout the placement, the Local Authority remains integral to the planning process, receiving regular updates through verbal communication and formal reports on a weekly or monthly basis. We, at Kezi & Clay Healthcare Ltd, actively seek and involve other services or professional support as necessary to meet the needs of the children under our care.

How referrals work

  • Transition and Support: Transitioning into a new living environment can be challenging for children. That’s why we provide comprehensive support services to ease the transition process.
  • Placement Matching: Every child is unique, and we strive to match them with the most suitable living environment within our residential home. Factors such as age, gender, additional needs, and personal preferences are carefully considered to ensure a positive and supportive placement experience.
  • Ongoing Evaluation and Feedback: We continuously evaluate our practice and the support we offer to ensure we meet the needs of the childen we care for.
